Please contact your insurance company to verify your physical therapy benefits using the insurance verification form and bring this completed, signed form to your first visit.
We conduct a thorough evaluation to determine the underlying sources of pain and dysfunction to ensure successful treatment. We also emphasize lifestyle chages that promote wellbeing, with an understanding of each client's unique set of circumstances.
Would you like a Good Faith Estimate?
If you do not have health insurance or plan to self-pay for our services, we will provide you with a "Good Faith Estimate” of our costs when you schedule at least 3 days in advance. If you would like to request a Good Faith Estimate before scheduling, please call our office at 207-324-3745.
